Mission Statement
The mission of the Murray City Risk Management Division is to create a safe working environment for all employees, cultivate and promote a safe atmosphere for the surrounding community, and to protect the assets and financial interests of the City from potential loss and liability through the implementation of a comprehensive and effective risk management program.General Claims Information
If you have suffered property damage or injury for which you believe Murray City is responsible, you may wish to file a claim with the City.
The City has contracted with Travelers as its claim administrator. It is important to understand that no employee or representative of the City can determine the City’s liability for damages or injuries until you have filed your claim and received an official response from Travelers. Although City employees may respond to the scene of an incident, they are there to gather information and to provide necessary assistance, but they do not have the authority to determine liability or make statements about the City’s responsibility for your injuries or damages. Also, the City has a financial responsibility to its citizens to handle all claims in a financially prudent manner as claim payouts come from taxpayer dollars.
To submit a claim, please print and fill out the claim form. Once completed, please sign and notarize the Claim Form and mail or email a copy as indicated below, along with any documentation supporting your claim. Relevant documentation might include, but is not limited to, police reports, witness statements, photos, repair estimates, invoices, etc. For property claims greater than three thousand dollars ($3,000), please provide a minimum of three (3) repair estimates. The City reserves the right to hire an independent appraiser for all claims. Under Utah state law, you must submit your claim within one (1) year after your claim arises.
Once we receive your claim, we will provide the information to Travelers to determine whether the City is liable for any damages. Travelers will contact you and will work with you directly to evaluate your claim. If there is a settlement or payment for damages, you will need to sign a Release before any funds can be paid to you.

FILE A CLAIM
To file a claim, please follow these steps:
- Download and print the Murray Claim Form (PDF). If you are unable to download or print a form, please contact the Murray City Risk Division to discuss alternatives for obtaining a Claim Form.
- Fill out the Claim Form completely and have it notarized.
- Attach any documentation in support of your claim (photos, repair estimates, police reports, etc.)
- Deliver the Claim Form and all attachments to one of the following:
