 Appointment Information

NOTICE: 

Murray City Passport office will be closed December 16 from 11:30 to 1. 

APPOINTMENTS REQUIRED! 

Book online to request an appointment to secure your spot.

Appointment times are every 10 minutes. If you are more than 5 minutes late you will need to reschedule.

Notice: There is a shortage of DS-11 Forms (New Applications). Before your appointment, please make sure your download a FREE Form from www.travel.state.gov.

  • APPOINTMENT: Please arrive early for your appointment & have your passport application filled out in BLACK ink or typed.
  • IMPORTANT: If an applicant makes an error  they MUST complete a new DS-11 form.  Corrections made with white-out, crossing out the incorrect information, writing the correction on top of the error, etc., are NOT acceptable.
  • NOTICE: Do not pay for Passport Forms (DS-11 or DS-82) online. You can find these forms for FREE at Travel.state.gov.
  • NOTE: Passport applications are accepted Monday through Friday from 8:00 am to 3:45 pm (closed legal holidays).  Passport applications will not be processed after 3:45 pm.
  • DO NOT SIGN the application until requested to do so by an acceptance agent.

Food Drive

Murray City is hosting a food drive this year to support the Murray Children’s Pantry. We are looking to gather food items that are non-perishable, non-glass and not expired. If you would like to donate, please see bring a non-perishable canned good. 

The drive will run from Monday, November 10th – Tuesday, November 25th (by noon).

Personal Appearance


All applicants (including children) must personally appear when applying for a passport.

  • Children under 16 years of age must have both parents present.
  • Children 16 and 17 years of age must have one parent present.
  • Social security numbers must be provided for all passport applicants.

Common mistakes

  • Page 2- Section 10 - Parental Information
    • When entering in Mother's LAST Name, it's asking for her name at birth (maiden name).
  • Page 2- Section 21 - Previous Passport
    • If you bring in your passport to the appointment, please mark "Submitting with application" not "In my possession".  The best practice is to send in your expired passport with your new application. You will get your old passport back.

Passport Photos

Passport photos are available in the Murray City Recorder’s Office for a fee of $15.00 per photo (payable to Murray City). Photograph requirements include the following:

  1. One U.S. Passport photograph is required with the application.
  2. The photograph must have been taken within the last six months.
  3. Pictures must be in color and on a white background.
  4. Pictures must be a 2 by 2-inch square

Passport Fees

Please note that you must bring two separate payments when applying for a U.S. Passport:

  1. The Application Fee needs to be in the form of a check or money order ONLY made payable to the U.S. Department of State
  2. The Execution Fee is payable to Murray City and can be made in the form of cash, check, credit, or debit card.

Please note: Passport cards are valid for land and sea entry only into Canada, Mexico, Bermuda, and the Caribbean. They cannot be used if you plan to fly to one of these destinations.

1-2 Day DeliveryPaid per application for 1-2 day delivery of an issued passport book from the Department of State to the customer. Only applies to mailing addresses within the United States. Not valid for passport cards. $22.05
File Search FeeA file search is necessary when an applicant is unable to present evidence of U.S. citizenship or verification of a previously issued U.S. Passport or Consular Report of Birth Abroad. Applicant must submit Form DS-11 and a written request for a file search.$150

General Passport Information

The Passport Book is used for international travel by air, land, and sea. The Passport Card is only valid when entering the U.S. by land and sea from Mexico, Canada, the Caribbean, and Bermuda. It is not valid for international air travel.

Once you have submitted your application, you can check the status of your passport application on the U.S. Department of State’s website. Please allow 2-3 weeks for your passport information to be loaded online.

PROCESSING TIME

Processing Updates from travel.state.gov 

Routine service can take up to 4-6 weeks (plus shipping time) from the day an application is submitted to the day a new passport is received. 

Expedite service (for an additional $60) can take up to 2-3 weeks (plus shipping time) from the day an application is submitted to the day a new passport is received. 

Processing times do not include mailing times. Mailing times vary across the country. Check your application status online at: passportstatus.state.gov. 

It may take up to 2 weeks from the day you apply until your application is "In Process".

Adult Online Renewals

Eligible U.S. citizens who want routine service can now renew their passports online. If you do not qualify to renew online, you may also renew by mail. Go to: travel.state.gov to see if you are eligible to renew your passport online.
