Those with Appointments will be helped first and walk-ins will be helped during gaps or no-shows.  WARNING: Walk-ins may have to wait 15 minutes or more and may be asked to schedule an appointment and come back another day. APPOINTMENTS

APPLICATION 

Please arrive early for your appointment & have your passport application filled out in BLACK ink or typed.

NEW IMPORTANT: If an applicant makes an error on the DS-11, they MUST complete a new DS-11 form.  Corrections made with white-out, crossing out the incorrect information, writing the correction on top of the error, etc., are NOT acceptable.

Common mistakes: 

• Page 2- Section 10 - Parental Information
  • When entering in Mother's LAST Name, it's asking for her name at birth (maiden name).
• Page 2- Section 21 - Previous Passport
  • If you bring in your passport to the appointment, please mark "Submitting with application" not "In my possession".  The best practice is to send in your expired passport with your new application. You will get your old passport back.

PROCESSING TIME

Routine service can take up to 8-11 weeks from the day an application is submitted to the day a new passport is received. 

Expedite service (for an additional $60) can take up to 5-7 weeks from the day an application is submitted to the day a new passport is received. 

Processing times do not include mailing times. Mailing times vary across the country. Check your application status online at: passportstatus.state.gov. 

It may take up to 2 weeks from the day you apply until your application is "In Process".

Process

Passports are processed by appointment (recommended).  Please have your applications filled out, in black ink, before coming to your appointment.  You can download the application form from  travel.state.gov, or pick up applications at City Hall.

DO NOT SIGN the application until requested to do so by an acceptance agent.

Passport Acceptance Facility

The Murray City Recorder’s office has been designated as a Passport Acceptance Facility by the U.S. Department of State. We are located at:

Passport applications are accepted Monday through Friday from 8:00 am to 3:45 pm (closed legal holidays).

Passport applications will not be processed after 4:00 pm.

Murray City offers services for Adult First Time Applicants, Minor Applicants, and Passport Renewals.

General Passport Information

The Passport Book is used for international travel by air, land, and sea. The Passport Card is only valid when entering the U.S. by land and sea from Mexico, Canada, the Caribbean, and Bermuda. It is not valid for international air travel.

Once you have submitted your application, you can check the status of your passport application on the U.S. Department of State’s website. Please allow 5-7 weeks for your passport information to be loaded online.

Passport Applications

Passport applications can be obtained and submitted through the Murray City Recorder’s office. The application is also available online.

In order to expedite the application process, we encourage you to download your passport application from the U.S. Department of State’s website and fill it out before coming into our office. DO NOT SIGN the passport application until requested to do so.  The Application is FREE on the US Dept of State's website. 

Please bring the following items with you when you come to apply for a U.S. Passport:

1. A check, money order, or certified check made payable to the U.S. Department of State for the application fee;
2. Cash, check, credit, or debit card to pay for your Execution Fees to Murray City;
3. Valid picture ID:

• Current Driver’s License
• Military ID
• FAA pilot’s license
• Certificate of Naturalization
• Government employee ID
• U.S. passport;

4. A copy of the front and back of your ID;

5. Proof of Citizenship:

• Original government-issued birth certificate (hospital-issued birth certificates are not accepted)
• Original Naturalization Certificate
• Original Citizenship Certificate
• Certificate of U.S. Birth Abroad
• Previous U.S. passport.

6. A copy of the front and back of your citizenship document.

Passport Photos

Passport photos are available in the Murray City Recorder’s Office for a fee of $15.00 per photo (payable to Murray City). Photograph requirements include the following:

1. One U.S. Passport photograph is required with the application.
2. The photograph must have been taken within the last six months.
3. Pictures must be in color and on a white background.
4. Pictures must be a 2 by 2-inch square.

Personal Appearance

All applicants (including children) must personally appear when applying for a passport.

Children under 16 years of age must have both parents present. Children 16 and 17 years of age must have one parent present.

Social security numbers must be provided for all passport applicants.

Passport Fees

Please note that you must bring two separate payments when applying for a U.S. Passport. The Application Fee needs to be in the form of a check or money order made payable to the U.S. Department of State. The Execution Fee is payable to Murray City and can be made in the form of cash, check, credit, or debit card.

Please note: Passport cards are valid for land and sea entry only into Canada, Mexico, Bermuda, and the Caribbean. They cannot be used if you plan to fly to one of these destinations.

Adult Renewals

To renew your passport, you must apply by mail or in person at a passport agency, if you are traveling within 14 days. A $60 expedited fee is charged at passport agencies. Below are the fees for basic renewals. Optional fees may apply.
