Fire Incidents Report
Fire Incident reports may be acquired by completing a records request form.
Medical Incident Report
Records containing protected health information as defined in 45 C.F.R., Part 164, are governed by HIPAA (the Health Insurance Portability Act) and are not subject to the Government Records Access and Management Act. Reports may be requested by completing a Request for Records form and will be released if they meet HIPAA regulations.
Environmental Records Requests
Record requests for environmental property assessments, directly associated with fire code violations, hazardous material spills, etc. can be obtained by completing a Request for Records form.

Submit completed forms to Laura Lloyd at llloyd@murray.utah.gov or by mail to Murray City Fire Department, 4848 South Box Elder Street, Murray, UT 84107. Each request will be processed to ensure compliance with HIPAA, which restricts release of information. Most requests can be processed within 7-10 business days. The cost of an incident report is $10.00 per copy.
